To bring it back online simply, sign-in as before and you can reactivate it by going to settings > My Xbox Live Account > Manage Profile > Delete All Unused Services. This recreates a new profile which then allows you to login again with your gamertag (same email address is used). For some people this has been enough to get their achievement problems fixed but others have had to go further with deleting or re-deleting entirely.

So you already tried all that and there’s no improvement? You might want to delete your profile entirely and start from scratch. This also avoids any chance of corrupt data in the database that could be causing you problems.
